DPSCD’s Health Hubs support the Whole Child Commitment to create a joyful, safe, and healthy learning environment for students and their families. Schools with an existing School Based Health Center (SBHC) through partnership with Ascension, Henry Ford Health, and Institute for Population Health aim to bring together existing and new programs that will serve students and their families throughout DPSCD.
By centralizing resources at neighborhood high schools, students and families across the district will have one point of contact, the Health Hub, to access resources by both DPSCD and its partners ensuring access is available and delivered with support and care. The Health Hubs will provide a consistent source to provide case management to enhance the experience for all DPSCD students and families.
The Health Hubs are another demonstration of the District’s commitment to the whole child, meeting the needs of every student and ensuring that each one of Detroit’s children has the opportunity and resources needed to learn.
Once the referral form has been submitted, a Health Hub Navigator will connect and coordinate needed services.

Families can benefit from their local school health hub by accessing a variety of helpful resources. Whether it's information on nutrition, mental health support, or workshops on healthy habits, these hubs are like one-stop shops for well-being. Hub Navigators will work with families to coordinate services as quickly as possible.
Expected Steps
- Identify your individual or family needs
- Complete the referral form requesting resources
- A Health Hub Navigator or External Service Provider will contact you
- Services and resources will be provided.
- The Health Hub Team will follow up to confirm
- A Health Hub Navigator will check in to monitor progress
