Health Hubs

DPSCD Health Hubs provide multiple resources for District families at specific neighborhood high schools across the District. All services are at no charge to District students and their families. Please share the information below with other DPSCD families so that everyone is aware of the resources available to support student attendance and success in school. We started to implement the Health Hubs last year and have already served thousands of families and students.
DPSCD Health Hub Resources

- Primary Medical Care - services through school-based health center partners; including immunizations, physicals, and basic medical care and checkups
- Mental Health Services
- Oral Health - screenings, preventative care, fillings
- Vision Services - screenings, free eyeglasses
- Basic Needs - all Health Hubs have a full Food and Resource Distribution Center with shelf-stable food items and basic needs (access survey intake form)
- Utility Support
- Housing Support
- Family Resource Gateway Survey - student or family member can complete a survey and receive immediate feedback with resources emailed directly to them. (access survey)
- Health Hub Navigator—a full-time staff person available to have a conversation to better understand needs and make the necessary connections to additional resources

How to connect with your Neighborhood Health Hub

Simply reach out to the nearest neighborhood Health Hub to speak with a Health Hub Navigator to set up an appointment to assist your student and/or family. Appointments are highly recommended to ensure that you receive the time and services that you or your student require. View location details and contact information.
The Health Hub Navigator will assist you in providing access to basic needs, health care, oral health, and connect you to additional services and resources. The Hubs are available to ALL students and families within the District.
