Welcome back to a new school year with Detroit Public Schools Community District!
We are delighted to have your family return for a new school year as a part of our community of exceptional parents and students.
As we begin a new year, we would like to collect some important information. It is critical for the District to have the most up to date contact information, health information, and permission forms from our students and families. Our school and our school district will use this information to stay connected throughout the year and to keep your students safe.
Schools will be distributing Back to School packets to returning students that did not already submit an updated enrollment packet to their school for the 2021-22 school year. Families should complete the packet and return the forms to their school as soon as possible. You can also download a copy of the packet on the right and print at home.
All forms should be completely filled out and signed where indicated. If you have already filled out forms in this packet in Spring or Summer 2021, please check with your school to ensure that they received your updated information.
Important Note regarding Enrollment Documentation:
If your address has changed, please provide updated “proof of address” documents to your school for review. If your student has updated immunization record, please provide it to the school. Families of returning students do not need to resubmit birth records.
