ATTIC Referral Form

ATTIC often receives a Referral for Evaluation from a school or teacher, sometimes at the parent's request. To maximize the referral process, discuss the following questions with school staff and parents:

  • What specifically does the student have difficulty learning?

  • How does his/her handicapping condition affect what he/she needs to learn?

  • What have we tried to accommodate the student? What modifications have we made to the curriculum, if any?

  • What seems to help the student learn best

    • With the answers to these questions in mind, print out and complete the following form and mail to ATTIC at 3700 Pulford, Detroit 48207.
