We are located at 3031 W. Grand Blvd., Suite 136, Detroit, Michigan, 48202 in the New Center One Building Welcome Center across from the Fisher Building.

Walk-ins are accepted until 2:30 pm on Monday, Tuesday, Wednesday, and Friday. Our hours of operation for walk-ins are 8:30 am to 2:30 pm. The walk-in counter is closed on Thursdays. However, the drop-box is available between 8:00 am – 5:00 pm.


Transcript Request Information

You must provide a copy of your identification in order to obtain a copy of your transcript records. The following documents are accepted as I.D.:

  • Driver's License
  • State I.D.
  • Passports
  • Social Security Cards
  • Birth Certificates
  • Court Documents
  • Utility Bills, Medical Records
  • Mail with Identifying Name and Address

If you are under 18 years of age, a parent or guardian must request your records.

If you are 18 or older, “you” must request the transcript or provide a written and signed documentation and a copy of your identification authorizing someone else to obtain a copy of your transcript.


How to Request a Transcript


In Person (Central Office only)

You may request a transcript record by coming into our office during our normal business hours.

  • Take the next available number at the counter
  • Take a transcript release form
  • Fill out the form and sign it
  • Submit the signed form at the counter when your number is called

In Writing (Email, Fax, Drop-Box, US Mail)

Pick-up or download the transcript request form.

If you are unable to download or pick-up the Transcript Release Request form, a written request may be mailed to the address below or faxed to the fax number below:

You may also place requests in the drop box located on the 1st floor of the New Center One Building in the lobby of the Welcome Center between 8:00 am and 5:00 pm, Monday thru Friday.

Your transcript cannot be released without your signature on the written request.


By Telephone

Requests for transcripts/records may be submitted by telephone by leaving all information listed below. In all cases, include the following information:

  • Your name
    (include all names under which your record may be filed)
    First, middle, (maiden) last name (last name on school records)
  • Parent Information - Parent/Guardian Name
  • Information being requested (e.g. copy of transcript, Proof of enrollment, Proof of graduation)
  • A current phone number (if available)
  • Your Date of Birth
  • Last Detroit Public School attended
  • Date or period of attendance
  • Graduation Date (if applicable)

Also, provide information on how transcript is to be forwarded: person/institution to receive transcript, their fax number if it is to be faxed, or the address where it is to be mailed.

Please allow 5-7 business day for information to be forwarded.


How to Request Other Student Records

If you require a Special Education record, please contact the Office of Psychological Services at 313-866-0458. Their fax number is 313-866-0656 attention Valerie Strong.

If you require a Code of Conduct record, please contact the Office of Student Assistance and Intervention at 313-873-4857 or 873-4858 or fax to 313-873-7971 attention Dr. Foster.

If you require an Adult Education or Practical Nursing Program record, please call the Office of Adult Education at 313-852-1120 or 852-1085.

If you require a G.E.D. record, please call the Office of G.E.D. at 313-596-7615 or 596-0872.
